Referral Form
Hyperbaric oxygen therapy is offered as a referral and consultation service. While patients are undergoing treatment, they remain under the care of the referring physician. Copies of treatment documentation are sent to the referring physician. Out of area patients may be referred to an appropriate local physician at the discretion of the referring physician. To refer a patient, simply download & print a referral form, complete the information, and fax back to the appropriate facility location. A patient can also be referred by calling any of our treatment facility locations. Adobe® Reader® is required to open the .pdf versions of the referral form on this page.
